Prior to your first visit, please complete the Hillsview Haven Client Form and sign the Release of Liability Form. While on the farm, you are required to wear closed toe boots. 

• If you are riding (coaching program only), your boot should have a quarter inch heel.

• If you are riding OR under the age of 12,  you are required to wear a proper fitting, certified riding helmet. 

Please complete the form prior to your first visit. All information within this form will remain confidential. Hillsview Haven provides equine assisted learning programs, where people learn through authentic experiences, reflection and building meaningful relationships with the horses. It is not a form of therapy. 

Measures are taken to ensure the safety of you and all members of the Hillsview Haven community, including our horses. However, horses can display unpredictable behaviour, resulting in inherent risk, either on the ground or in the saddle.

Please read the form carefully. If you agree to the terms, please print it off, complete all fields and submit it during your first farm visit.
